THE ATTRACTION IS A HIGHLY SENSORY EXPERIENCE THAT INCLUDES FLASHING IMAGES AND LIGHTS THROUGHOUT. THIS MAY BE A CONCERN FOR THOSE WHO MAY OR HAVE SUFFERED FROM SEIZURES OR EPILEPSY.

I consent to take part/any person in my care under the age of 16 taking part in activities at Urban Playground. In providing my consent I declare that I understand Putters activities themselves are not completely free from risk and I am responsible for my own actions when using the equipment. I accept that whilst Urban Playground staff will take the appropriate precautions to prevent accidents, this can never be guaranteed. I confirm that I am/the person under 16 attending with is physically fit and healthy and I undertake to advise Urban Playground of any change in this regard. I give consent that in the event of any illness/accident any necessary treatment can be administered to me. I confirm that I have read through the Activity Rules and agree to abide by the Activity Rules as laid out by Urban Playground and to follow the instructions of the Urban Playground staff at all times. I declare that the information submitted is correct and if any information changes I will notify Urban Playground. If a player is under the age of 16, I am authorised to confirm under the laws of England and Wales the player’s participation in said activity and accept they are under my supervision at all times. All users agree that they shall not participate in any activities provided by Urban Playground unless they are medically fit to do so.

If you have been in contact with or have demonstrated any symptoms of Covid-19, please refer to the latest government guidelines regarding when it is safe for you to attend activities such as those at Urban Playground. We will be following the same Government guidelines and reserve the right to potentially refuse access to anyone based on this. No refunds will be provided. We will be following government guidelines as & when they update and will communicate any changes to our procedures as you enter the premises. It is your responsibility to be aware of what these changes are and we ask that you follow these guidelines. We may ask you to leave the premises if you do not adhere to the government guidelines whilst on site prior to participating. Users will remove jewellery, mobile phones and other valuables. Participants will not eat whilst participating in activities provided Urban Playground. Participants will at all times only carry out activities that are within their skill level and of which they are able to retain control at all times. Participants will at all times follow the instruction of Urban Playground staff. Participants will not attempt to use equipment other than for its intended purpose. Participants will not attempt to climb or dismantle any props or equipment at Urban Playground. The Attraction includes vigorous activities and a high-tech mini golf course that has steps and bespoke features.  By attending the Attraction you accept that you have a duty to take reasonable steps to ensure your own safety, taking into account any personal medical conditions. Care and attention must be paid in the venue to avoid trips and falls. If pregnant, participants confirm that they will only undertake activities that are safe to do so. Participants confirm that if they have any sort of medical condition that they believe may affect their ability to safely utilise the activities, they must consult a qualified medical practitioner before participating.
